1. Novel patient behavior profiling - I built a custom data science technique that analyzes over 13,000 patient voices to build detailed behavior profiles, capturing each patient’s goals, motivations, and challenges. This helps in designing targeted interventions and support strategies for clinicians. 2. Agentic AI powered clinician dashboard - I designed and built an intelligent clinician dashboard that analyzes patient-reported data, clinical information and lab reports to generate personalized plans of care. Clinicians gain fast, actionable insights and tailored treatment plans just at the click of a button while retaining clinician autonomy. 3.Intelligent patient app for ambient data capture - I designed an intelligent interface for patients that helps capture behavioral data points such as mood, symptom trends and social well being factors. This helps in designing context aware interventions and care plan generation.

Overview

Traditional care pathways often focus narrowly on clinical metrics, overlooking the human aspects that shape a patient’s daily life. The Reimagining Care Pathways project brings emotional, social, and quality-of-life factors to the forefront of treatment design in the form of a new vision for 2040.

Through a patient-friendly app, individuals can log moods, share goals, and reflect on life challenges, generating rich human-centered data. An AI-powered clinician dashboard synthesizes this information alongside clinical data, highlighting trends in emotional well-being and social needs. This empowers care teams to craft dynamic, personalized care plans that honor the patient’s lived experience, resulting in better outcomes and more compassionate, holistic care.

Problem

The healthcare system is under pressure. There are more number of patients than the number of healthcare professionals. Moreover, chronic care is currently fragmented, with generic clinical guidelines and a one size fits all approach.

  • Chronic patients get ~ 10-15 Mins per consultation

  • Clinicians are burdened with increased ↑ administrative workload

  • NO time for Clinicians to personalize care for every single patient

  • Declining ↓ patient adherence and poor health outcomes

Solution

I designed a new way for augmenting clinician workflows with AI to co-create personalized and explainable care plans. I designed an AI-powered clinician dashboard that generates a personalized plan of care based on behavioral, clinical and lab reported data. It consists of the following features :

  • Generates a personalized plan of care using clinical and human metrics

  • Enables modular assignment of clinical interventions to the care team

  • Translates medical jargon into patient-friendly language

  • Provides clinicians with full control and autonomy over AI-driven suggestions

  • Non-MDR, non-regulated decision support system

Impact

The new clinician-facing dashboard provides real-time behavior insights, for 2x faster decision making.
